Appointment
Appointments are an important part of your treatment and this time has been reserved specifically for you. Please arrive on time for your appointment. If you are more than 15 minutes late, you may be asked to reschedule. We request a 48-hour notice if you need to cancel or reschedule your appointment. We are aware that unforeseen events sometimes require missing an appointment and we appreciate your cooperation.

Payment
Total Pain Care strives to provide you with affordable, high-quality treatment that fits your budget. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We accept a variety of payment options, including cash, checks, and Visa, MasterCard, American Express and Discover credit cards. We will thoroughly explain your choices and work hard to accommodate your financial needs. If you have a deductible, it will be due on the date of service.

Insurance
Our office staff will be glad to assist you with your insurance coverage. You must bring your insurance cards with you on your first visit. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
We are in-network for most major carriers and we will file all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
